The four build tiers
The Site tier is $2,500. The doctor records a two-minute Loom about a clinical topic and it becomes an indexed article. This is where most growing practices land after the Site-tier conversation.
The Engine tier is $9,000. That is the programmatic build for multi-location dental groups. Up to 200 pages spanning per-procedure, per-location, per-insurance-carrier, and per-symptom, with schema automated across the entire graph. If you run three or more chairs across two or more locations, this is the math that works. The Studio tier is $15,000 and up. Custom builds for DSOs and dental groups that need a patient portal, an embedded booking layer beyond a third-party widget, or a clinician-facing intake tool. The four retainer tiers
Care is $60 a month. That covers Cite-met hosting, llms.txt and schema maintenance, and monthly AI-traffic tracking across six AI engines. If your clinic is small enough that the build is the bulk of the work and you do not need monthly content, Care is the floor.
Local is $300 a month. That is Care plus one SEO post a month, local schema upkeep, and Google Business Profile sync. Most single-location practices land here. The post is usually a clinical topic the doctor cares about, recorded as a Loom and turned into a 1,200-word article with the right schema.
Growth is $700 a month. That is Local plus four posts a month, brand-mention tracking across AI engines, and light link building. Most two-to-three location groups end up here after the first six months.
Authority is $2,000 a month and up. That is Growth plus AEO citation-building, Reddit and Wikipedia seeding, digital PR, and original research. Reserved for DSOs and groups that want their clinicians cited by name across the AI engines for clinical topics, not just proximity searches.
What about the $1,400 a month agency we use now
